WRMS Student Athletes need to complete the following before participating in athletic competitions. 

Register your child for sports on Power Schools: https://bcsd.powerschool.com

Sign in to the parent portal --> Forms --> General Forms --> 25-26 Athletic Forms  

• Sports Physical - You need this every two years.  If you get one in 6th grade you will have to get another in 7th grade.  Then you will not need another one until your Freshman year.
• Student Code of Conduct Signatures- This needs to be filled for every sport
• Paid Activity Card - This is necessary if you are an athlete or in and activity.
• Interim Health Questionnaire
• Impact Testing (Concussion Baseline)  This is necessary once a year.  Please check the date of your last Impact test Enter this website into your browser-https://www.impacttestonline.com/htmllauncher-Enter this code-Huw3zsqk6v
• Athletic Activity Release:  (optional)
• Proof of Insurance or purchase Insurance (see below)
• Student Insurance (all students must have health insurance.  If your student needs health insurance, you can purchase it through K&K Insurance.  

**IF A STUDENT DOES NOT ATTEND WRMS, THEY MUST HAVE COMPLETED THE DUAL ENROLLMENT PROCESS BEFORE THEY ARE ALLOWED TO PARTICIPATE**
