WRMS Student Athletes need to complete the following before participating in athletic competitions. 

Register your child for sports on Power Schools: https://bcsd.powerschool.com

Sign in to the parent portal --> Forms --> General Forms --> 25-26 Athletic Forms  

• Sports Physical 
• Student Code of Conduct Signatures
• Paid Activity Card
• Interim Health Questionnaire
• Impact Testing (Concussion Baseline)
• Athletic Activity Release:  (optional)
• Proof of Insurance or purchase Insurance (see below)
• Student Insurance (all students must have health insurance.  If your student needs health insurance, you can purchase it through K&K Insurance.  

**IF A STUDENT DOES NOT ATTEND WRMS, THEY MUST HAVE COMPLETED THE DUAL ENROLLMENT PROCESS BEFORE THEY ARE ALLOWED TO PARTICIPATE**
